No call, no nights, Sundays and major holidays off.


Full time position available for a Nurse Practitioner who is competent in performing risk stratification assessments for major insurance companies within the Commercial, Medicare, and Medicaid populations.


You will be working on a mobile unit. This position requires travel throughout Alabama, which will occasionally be overnight. Travel expenses/mileage are paid.


Expect 10-12 hour work days. Three days per week is considered full-time.


Paid training is provided to assist you in targeting long term chronic diseases such as congestive heart failure, COPD, diabetes, PAD, neuropathy, etc., in accordance with HCC, HEDIS, STARS and quality measures in a mobile clinic setting. A variety of point of care testing such as labs, ultrasound, spirometry, ABI and EKG are immediately available to validate old conditions or diagnose new conditions affecting the long term health of our patients. These test are performed under the supervision of the Medical Director, and in conjunction with a team of cardiovascular technologists and medical assistants on the bus.








The Nurse Practitioner (NP) provides comprehensive care to patients of variying ages. Attends to the patient’s needs with utmost patience, care, compassion and understanding. Performs tasks such as conducting tests, recording medical histories, assisting physicians and enhancing public awareness on the importance of their overall health and medical conditions.


Educational Requirements



  • Master's or Doctoral degree in nursing
  • Completion of a state acceptable Nurse Practitioner's program
  • National certification in specialty is required
  • Licensed to practice by the state in which you will be working
  • Collaborative Agreement will be with an MD/DO licensed by the state in which you will be practicing, unless State statute allows for independent practice by the NP



Required Skills and Abilities:


  • Required 1-3 years of clinical working experience.
  • Considerable knowledge of interviewing and counseling techniques.
  • Ability to work independently, communicate effectively both verbally and in writing
  • Deal tactfully and patiently with clients
  • Establish and maintain records, reports and statistical data
  • Use initiative and judgment
  • Maintain effective working relationships with other related agencies, professionals and citizen groups
  • Must be proficient in the use of medical and laboratory equipment
  • General knowledge of HCC measures ICD-10 codes is a plus
  • Experience with electronic medical records (EMR)


Supervisory Responsibility: None


Travel Requirements: Employee may be asked to travel within licensed state overnight.  Expected to be at the designated location on time accordingly to the schedule that has been provided by the manager.



Physical and Psychological Demands:

  • Requires prolonged sitting due to excessive driving
  • Requires bending, stooping, twisting, kneeling, crouching, crawling and/or stretching from seated or standing positions
  • Requires eye-hand coordination and manual dexterity sufficient to operate medical equipment, computer tablet, computer keyboard, telephone, copier, calculator and other office equipment
  • Requires close vision, peripheral vision, and ability to adjust visual focus, hearing and smelling
  • Must be able to communicate information via telephone or computer
  • Requires moderate to intense concentration due to complexity of the job
  • Must be able to lift and/or move up to 75 lbs.
  • Must be able to observe “Universal Precautions” for blood-borne pathogens and body fluids
