    Responsibilities

    $60 per hour.

    No call, no nights, Major holidays off. Saturday rotation schedule. 3-12 hour shifts. Advancement opportunities available in the areas of auditing, training, and management. Some Sundays required.

    Immediate need for a Full time position Nurse Practitioner who is competent in performing risk stratification assessments for major insurance companies within the Commercial, Medicare, and Medicaid populations. 

    The NP will be working on a mobile unit covering areas throughout the State of Ohio--including Columbus, Dayton, Cincinnati, and Cleveland.

    Paid training is provided to assist in targeting long term chronic diseases such as congestive heart failure, COPD, diabetes, PAD, neuropathy, etc., in accordance with HCC, HEDIS, STARS and quality measures in a mobile clinic setting. A variety of point of care testing such as labs, ultrasound, spirometry, ABI and EKG are immediately available to validate old conditions or diagnose new conditions affecting the long term health of our patients. These test are performed under the supervision of the Medical Director, and in conjunction with a team of cardiovascular technologists and medical assistants on the mobile unit.

    Qualifications

    Education

    • Bachelor of Science in Nursing preferred
    • Master’s Degree required OR required commensurate experience and satisfactory completion of NP licensure

     

    Licensing/Certification:

    • Current RN (if required) and NP licensure in state of practice
    • Board certification by American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or American Association of Critical Care Nurses (AACCN) as an Acute Care, Adult, Adult-Gerontology, Family, or Geriatric, or other approved specialty
    • Current Basic Life Support (BLS) certification, which may include Adult, Child, and Infant Cardiopulmonary Resuscitation (CPR)
    • General knowledge of HCC measures and International Statistical Classification of Diseases and Related Health Problems (ICD-10) codes a plus

     

    Required Skills and Abilities:

    • One (1) to three (3) years of clinical working experience as a Nurse Practitioner in geriatric, adult or family practice setting.  

    Travel RequirementsTravel is required in state of practice and overnight in-state travel may be required
