    Position: Medical Assistant (MA), Home Health Aide (CHHA), Nursing Assistants (CNA)

    Reports to: Clinical Manager

    Department: Clinical Operations

    Status: Contracted 1099 Per Diem

    Location: Various

Using a patient-centered approach, our providers seek to understand the current state of the members’ health and support them in setting mutual, realistic goals. As a progressive leader in Wellness Assessments, we are seeking credentialed, reliable, and caring CMAs, CHHAs or CNAs providers that are interested in working in the Wellness Assessment field. We are offering IMMEDIATE Per Diem, 1099 positions across the United States to attempt to locate members of Health Plans who, if located, are in need of a short Health Risk Assessment. Delivering quality and convenient healthcare requires a passion for understanding members’ needs and subsequently assisting them in taking preventative measures to mitigate current or future health risks.     The contracted Provider is expected to demonstrate proficient skills that support our member “Wellness Assessments”. The position may include work in a clerical, administrative, or clinical assessor capacity, and may be asked to perform more specialized tasks. CMAs, CHHAs, CNAs (or similar) provider types will attempt to locate members of Health Plans who, if located, are in need of a short Health Risk Assessment. Providers will attempt to contact by telephone, and knocking on doors to try to make contact with the member. Once the member is located, a Health Risk Assessment will be conducted.


    ï‚· Welcome members by greeting them, in person or on the telephone

    ï‚· Verify member information by interviewing member; reviewing and recording medical history; taking vital signs if required; confirming purpose of visit

    ï‚· Provide appropriate member education on medication, diet information and instructions; ensures questions are addressed or referred within scope of certification

    ï‚· Complete records by recording member assessment information and emailing to a secure email encryption program *Laptop computer with Windows 7 or above or OSX 10.9 or above

    ï‚· Maintain safe, secure, and healthy work environment by following, and enforcing standards and procedures; complying with legal regulations

    ï‚· Maintain member confidence and protected health information (PHI) by keeping member care information confidential

    ï‚· Adhere to professional standards, and Matrix Medical Network policies and procedures, federal, state, and local requirements

    ï‚· Completes assessments

    ï‚· Other duties as required



    Educational Requirements:


    ï‚· High school diploma or equivalent


    ï‚· Current certification as an MA, HHA or NAs (or similar) certification or degree preferred

    ï‚· Maintains current CPR certification


    Required Skills and Abilities:

     One (1) to two (2) years’ experience in the healthcare field and medical specialty

    ï‚· Knowledge of medical terminology

    ï‚· Highly effective interpersonal and communication skills; ability to communicate to all levels throughout the organization using written, verbal and listening skills

     Demonstrated ability to accurately document a member’s medical history and to report technical information to other doctors and patients; establish and maintain records, reports and statistical data

    ï‚· Outstanding teamwork and multitasking skills. Ability to thrive and prioritize in a dynamic, rapidly changing environment

    ï‚· Dedicated to meeting the expectations and requirements of both external and internal customers, acts with customers in mind by exhibiting professionalism, empathy and courteousness

    ï‚· Passion for working with members/patients, changing lives with a comforting bedside manner

    ï‚· Consistently demonstrate a high level of professionalism, demeanor and confidentiality by using initiative and judgment

    ï‚· Demonstrated ability to meet deadlines, and use time effectively and efficiently. Concentrates efforts on priorities with detail and strong organizational skills. Ability to maintain effective and organized systems to ensure timely patient flow

    ï‚· Participate in educational opportunities to remain knowledgeable of position requirements related to job performance; reading professional publications and participating in professional organizations.

    ï‚· Proficient in Microsoft Office applications, including, Outlook and Word


    Supervisory Responsibility: No supervisory responsibilities required

    Travel Requirements:

    ï‚· Must meet company driving standards

    ï‚· Must have valid state driver license and be able to drive a car

     Must meet company’s automobile liability insurance standards and provide proof of coverage

    ï‚· Must be able to independently travel to assigned locations


    Physical and Psychological Demands:

    ï‚· Requires prolonged sitting, bending, stooping, kneeling, and/or stretching from seated or standing positions

    ï‚· Requires eye-hand coordination and manual dexterity sufficient to operate medical equipment, computers other office equipment

    ï‚· Requires close vision, peripheral vision, and ability to adjust visual focus, hearing and smelling

    ï‚· Must be able to communicate information via telephone or computer

    ï‚· Requires moderate to intense concentration due to complexity of the job

    ï‚· Must be able to lift and/or move up to 50 lbs.

     Must be able to observe “Universal Precautions” for blood-borne pathogens and body fluids
