    Skilled Nursing Facility Representative

    The Facility Relations rep performs daily analysis for the Skilled Nursing Facilities (SNF) program to assist in CHA completion. They will make phone calls to multiple facilities daily to confirm and verify members are still residents. They will also work directly with Production Planning, Clinical Operations, IT, Marketing, and the Contact Center in order to optimize provider productivity and to ensure all key performance indicators are on track to achieve business and client objectives.  Clerical and administrative duties will be necessary to ensure the continuing operation of the SNF Program. 


    • Verify members are still residents in SNFs, up to 100 calls daily
    • Obtain room numbers, Hospice status, Power of Attorney (POA) info on verification calls
    • Utilize Geographic Information System (GIS) techniques to provide a better understanding of variables in a given geographic location
    • Processes changes to Facility assignments
    • Data entry into company database
    • Performs other administrative tasks and duties as necessary.


    Educational Requirements:

    High School Diploma or GED equivalent.


    Required Skills and Abilities:

    • Two years experience in a fast-paced and production oriented business office, healthcare environment preferred.
    • Ability to speak to SNF Employees and gain information
    • Strong Analytical Skills
    • Familiarity with HIPAA rules and regulations
    • Intermediate proficiency in MS Office, with emphasis in Excel
    • Problem solving skills with ability to identify situations requiring the attention of management
    • Demonstrated success in a team environment and effective communication skills interacting with both internal and external customers
    • Strong attention to detail and effective organizational skills


    Supervisory Responsibility: No supervisory responsibilities
